We are always interested in expanding our provider network to provide care to an even greater amount of care to Nashville area patients. Anesthesia Provider
Tennessee Surgery Center holds patient safety in the highest regard. Patients are offered a variety of anesthetic options while undergoing care. Anesthesia providers are present to ensure both patient safety as well as provide the appropriate anesthetic environment for procedures performed at Tennessee Surgery Center. We encourage new anesthesia providers (M.D. or C.R.N.A.) to apply for initial privileges download Initial Application-CRNA and returning it to:

There is a $150 initial credentialing fee for Tennessee Surgery Center which is required by state guidelines and to facilitate the arduous credentialing process. If you have previously had privileges at Tennessee Surgery Center and need a re-credentialing application, you may download Re-Credentialing-Application and the fee is only $100. Please include a check, in the appropriate amount, made out to “Tennessee Surgery Center” with your credentialing application. Dentist : Pediatric Dentist Application for the Tennessee Surgery Center is proud to be one of the only AAAHC accredited facilities in Middle Tennessee to actively provide dental rehabilitation in a setting appropriate for your special needs patients. As a Registered Dental Assistant (RDA), you are a vital member of your office’s ability to provide care to patients here at Tennessee Surgery Center. To promote the atmosphere of safety and accountability that is required at our AAAHC accredited facility, state laws require all Registered Dental Assistants (RDA’s) and Dental Technicians be appropriately credentialed prior to assisting their respective Dentists.
